Semaglutide

Semaglutide is a medication originally developed to treat type 2 diabetes, but it has gained significant attention for its effectiveness in weight loss. It is a GLP-1 receptor agonist, meaning it mimics a hormone called glucagon-like peptide-1 (GLP-1), which helps regulate blood sugar, slow digestion, and reduce appetite.

How Semaglutide Helps with Weight Loss

  • Appetite Suppression: It signals the brain to reduce hunger, leading to lower calorie intake.

  • Slowed Gastric Emptying: Food stays in the stomach longer, making you feel full for an extended period.

  • Blood Sugar Regulation: Helps stabilize blood sugar, preventing cravings and overeating.

FDA-Approved Versions for Weight Loss

  • Wegovy: Approved specifically for weight management in individuals with a BMI ≥30 (obese) or BMI ≥27 (overweight) with at least one weight-related condition (e.g., hypertension, type 2 diabetes).

  • Ozempic: Approved for type 2 diabetes but often prescribed off-label for weight loss.

Effectiveness

Clinical trials have shown that semaglutide can lead to an average weight loss of 10-15% of body weight over 68 weeks when combined with diet and exercise.

Dosage and Administration

  • Injection:

    • Typically starts at 0.25 mg once a week, gradually increasing to 2.4 mg every 4 weeks (for weight loss).

    • Administered as a subcutaneous injection in the stomach, thigh, or upper arm.

  • Oral Tablet

    • Starts at 2mg daily, gradually increasing to 4mg daily every 4 weeks.

Side Effects

  • Common: Nausea, vomiting, diarrhea, constipation, bloating.

  • Serious: Risk of pancreatitis, gallbladder disease, kidney issues, and possible thyroid tumors (black box warning for medullary thyroid carcinoma).

Who Should Avoid Semaglutide?

  • Those with a history of medullary thyroid cancer or multiple endocrine neoplasia syndrome type 2 (MEN2).

  • Individuals with a history of severe gastrointestinal disease or pancreatitis.

  • Pregnant or breastfeeding women.

Cost and Availability

  • Expensive without insurance (~$1,000+ per month) for branded semaglutide.

  • Insurance coverage varies; some plans cover Wegovy for obesity treatment.

Final Thoughts

Semaglutide can be a powerful tool for weight loss, but it should be used alongside healthy lifestyle changes for long-term success. If you’re considering it, consult with a healthcare provider to determine if it’s right for you.
